A Registered Nurse (RN) is required to make the initial evaluation visit. The RN must initiate the plan of care and make necessary revisions to your plan of care as needed. A Licensed Practical Nurse will be assigned to you and perform the necessary education and treatment ordered by your physician.
Skilled Nurse services are designed to educate the patient, caregiver, or family member on specific medical issues that you have.
Some examples are:
PICC line maintenance and infusions
Disease process instruction
Foley catheterization insertion and maintenance
Monitor labs for new medications. Example: Coumadin, antibiotics
Educate on infection control, fall prevention, etc.
The goal for Skilled Nursing Services is to educate the patient, family member or caregiver on tasks/services that are required after hospital admission, change in the patient's medical conditions, or new diagnosis of a medical condition.
If a patient is a Physical Therapy only patient, the RN will still make the initial evaluation for the patient. After the initial evaluation, a Physical Therapist (PT) will be out to do an evaluation and set up a program to meet your individual needs. Once a program is set up for you, a Physical Therapy Assistant (PTA) will be assigned to you to assist you in your personal therapy program.
