Physical Therapy Services

A Registered Nurse (RN) is required to make the initial evaluation visit. The RN must initiate the plan of care and make necessary revisions to your plan of care as needed. A Licensed Practical Nurse will be assigned to you and perform the necessary education and treatment ordered by your physician.

Skilled Nurse services are designed to educate the patient, caregiver, or family member on specific medical issues that you have. 

Some examples are:

Diabetic Education

Wound Care

PICC line maintenance and infusions

Nutrition, diet

Medication Safety

ZOE Monitoring

PT/INR Monitoring

Disease process instruction

Foley catheterization insertion and maintenance

Monitor labs for new medications. Example: Coumadin, antibiotics

Educate on infection control, fall prevention, etc.

The goal for Skilled Nursing Services is to educate the patient, family member or caregiver on tasks/services that are required after hospital admission, change in the patient's medical conditions, or new diagnosis of a medical condition.

Physical Therapy Services

If a patient is a Physical Therapy only patient, the RN will still make the initial evaluation for the patient. After the initial evaluation, a Physical Therapist (PT) will be out to do an evaluation and set up a program to meet your individual needs. Once a program is set up for you, a Physical Therapy Assistant (PTA) will be assigned to you to assist you in your personal therapy program.

Criteria for therapy include:

  • Pain
  • Gait disturbance
  • Joint surgery
  • Weakness
  • Inability to perform activities of daily living
  • Stroke

Some modalities of treatment may include:

  • Anodyne therapy
  • Ultrasound therapy
  • TINS therapy
  • Therapeutic massage
  • Pre-operative joint education
  • Pain management techniques

A client will need speech therapy for the following:

  • Difficulties with swallowing
  • Difficulties speaking as a result of a new CVA (stroke), multiple sclerosis

Occupational Therapy Services

A client will need occupation therapy when they have a diagnosis of:

  • Carpel Tunnel
  • Wrist fracture
  • CVA (stroke)
  • Hand Pain
  • Multiple sclerosis
  • Arthritis
  • Parkinson's
  • Traumatic brain injury

Other Conditions:

  • A patient needing maximum assistance with self care that is recent, at least within the past two (2) months
  • Severe swelling in hand or upper extremities
